Chris was a resident with me at NYU medical center. We started a group for homeless teenagers at Covenant House (pre scandal) and became good friends. Even the last time I saw him, accompanying him to his medical visit, though he wasn't feeling well, he showed his humor and strength. I will think of him when I face the end, to emulate his bravery and graciousness.

I am very glad to have been friends with Chris during our time in Madison in Med School. I just happened to be trying to look him up on facebook today and then came across this notice. I am very sad to hear about this. He was always such a sweet person and a lot of fun at his parties and in group trips to The New Bar for dancing. A great loss.

My condolences go out to Dr Chris's family, to Scott and Cory. I knew Dr Chris for a very long time. He was my mothers Dr and when I was old enough to go to the Bars I met Dr Chris and Scott. Chris always had a Smile on his face and always seemed Happy! I will never forget when I DJ at the SKy bar for Scott and Chris and Chris would always come in on a Friday or a Sat night and ask me to play this song called "Juicy Fruit" He would go out on the dance for and CUT A RUG! He had great moves and was a great dancer. It saddens me that I never got to say good bye to Dr CHris but Chris I just want to say that MY MOTHER truly loved you and she will be lost without you! REST IN PEACE my DEAR FRIEND!!

It was my distinct privilege to work closely with Chris while serving on the Board of the Reg Lenna Civic Center. His passion for the arts was infectious and his ideas were contagious. I loved his thoughtful counsel and concern for our community and admired the work that he and Scott did to make our community a more beautiful place to live.I have no doubt he is quietly but doggedly making Heaven better and brighter as well.
Rest in Peace my friend...
and thank you to his family and friends for sharing him.

I will forever hold dear the memories of growing up with Chris and the times we spent in Green Lake during his summer visits to his Grandparents. His laughter and smile will stay with me always. I am grateful to have had the chance to see and visit with Chris over the past couple of years. He was not only my cousin, but a wonderful friend. Chris was an amazing man and will be missed by so many who lives are that much brighter for having known him. The strength, courage, and detimination he displayed during his battle with cancer is an inspiration.
Ardis, Al, Jack, Nancy and your families, my thoughts are with you all.

My sincere love and prayers to all family and friends of Chris as you mourn his loss. I enjoyed getting to know Chris as a business associate for many years. We had some great times, especially a nice evening at dinner during the APA in Atlanta in 2005. Our relationship grew even closer when later that year my late husband was diagnosed with cancer and Chris the year after that. We would have wonderful discussions about treatments, life and living with cancer. I will never forget the heartfelt hug I received from Chris when I returned to work after the loss of my husband. He was such a passionate and caring man and it grieved him deeply when he heard of my loss. I know it was a long 6 years for him and he was so dedicated to his treatment. He was so excited when he hit the 5 year mark. He will be missed and I am so grateful to have shared some of his short life with him. May God's Peace be with you all.
Love,
Julie Dahl

Chris was my supervisor for the past year and a half. He graciously agreed to take me on and supervise me as a new nurse practitioner. Having been in the mental health field for 30 years, I wasn't new to the field, but to my new role. I was so struck by Chris' humanity and caring for his clients and for all people. Chris believed in the dignity and worth of all human beings, and projected that to staff and to clients alike at all times. There was something beautiful and gentle about his spirit and soul - a deep sweetness that seemed to touch everyone that he met. I was awed by his ability to continue to do what he did best - care for others; despite his illness and he did it with such elegant grace and dignity. I feel blessed to have known him. He helped me grow in my new field and always cared and supported me every step of the way. What a rare, beautiful, dignified and gracious person he was.

I had seen Dr. Kaz off and on for seventeen years. He never tired of me and always tolerated my inconsistencies. He was a very empathetic person and perhaps one of very few people in my life that trusted my judgement though he wasn't always pleased with it. I admired him especially in the last few years because he never let his illness interfere with his ability to be a devoted doctor. He encouraged me to write and because of his support and others involved in my treatment I eventually completed my book. I had promised him a copy at our next appointment unfortunately that appointment never came. He was a gift to me as well as the community. It's hard to adjust being sent to a new doctor who has no sense of who I am but if there is one thing that I learned from Dr. Kaz it's face life head on with dignity no matter what it throws at you. I will continue to write and continue to acknowledge Dr. Kaz through my literal journey and hope that one day I can touch even a portion of people's lives like Dr. Kaz has mine. You will be deeply missed and always remembered. God bless your family and friends and may the memory of your proud inspiration carry them through this difficult time.

Wow Dr. K., you amazed me with your strength and determination to live each day no matter HOW you felt! You kept giving and working. Those two things are synonymous as your work was giving to others, listening, and helping...yet you were suffering as well. You seemed to have an endless supply of patience and empathy- kind ness to treat people with. Your battle was not in vain, Dr. K. It gave you more time to do your life's love. May you rest in peace now and know you have done the world well.

Dr. Kaczmarek changed my life! Forever! I became a patient of his three years ago. I have a mental health disorder and Dr. Kaczmerk was the 1st and only psychiatrist who saw me as a person. I was always trying to use my illness as a crutch in life...during one session I told Dr. K I just do not think i can work...I read the book you gave me on Bipolar and I should not be working. Dr. Kaczmarek's response to me was, "...so the book told you that you could not work?" To him I replied well, no no it did not. He inspired me so much and dispite my disability I felt he never saw that in me. Thanks to his impact in my life I have an Associates Degree in Applied Science - Nursing and am now attending JBC for a degree in Medical Administrative Assistant - transcription. Without him in my life those two things likely would never have come to be. Thank You Dr. Kaczmarek I will forever miss you.
